1. DO NOT submit PROOF OF AGE (POA) to our front desk. You MUST possess POA for each player, at each game, or risk forfeiture. Acceptable items: copy of player card (MSYSA/USYS), ID or copy of birth certificate. Failure leads to inability to play.
2. Soccer Dome ROSTER FORM (found on the "YOUTH LEAGUES" webpage) MUST be turned in to the front desk prior to the kick-off of your 1st game.
